Majors: megakaryocyte morphology, not another myeloid neoplasm, JAK2/CALR/MPL or other clone or no reactive fibrosis. Minors: anemia not from comorbidity, WBC ≥11e9/L, splenomegaly, LDH above ULN; leukoerythroblastosis counts only in overt PMF.
